HomeMy WebLinkAbout1975-119 CCR Member Bill Fignar introduced the following resolution and moved its adoption: RESOLUTION NO. 75 -119 RESOLUTION AWARDING INSURANCE CONTRACT WHEREAS, the Director of Finance has prepared specifications and sought proposals for certain City insurance coverages; and WHEREAS, these coverages were: A. Group Life and Accidental Death and Dismemberment Insurance for Permanent City Employees. (City Paid) B. Voluntary Group Life and Accidental Death and Dismemberment Insurance for Permanent City Employees. (Employee Paid) C. Voluntary Group Life and Accidental Death and Dismemberment Insurance for Dependents of Permanent City Employees. (Employee Paid) D. Group Life and Accidental Death and Dismemberment Insurance for Volunteer Firemen. (Paid by Volunteer Firemen); and WHEREAS, the Director of Finance and the City Clerk have reported that said proposals were received until and opened at 11 :00 a.m. June 17, 1975; and WHEREAS, said proposals were evaluated by the Director of Finance and the City Manager; and WHEREAS, they have recommended that the proposal submitted by the North American Life and Casualty Company as being the best as to coverage, service and price to the City of Brooklyn Center: NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of Brooklyn Center to accept the proposal and award an insurance contract for under- writing of a group life insurance program for the employees of the City of Brooklyn Center commencing August 1, 1975, to the North American Life and Casualty Company, J. Beryl Clifford and Associates, agent, as follows: RESOLUTION NO. 75 -119 Coverage Net Monthlv Cost A. Group Life Insurance (City Paid) $5, 000 through December 31 1975 Per $1 000 .48 $10, 000 effective January 1 1976, Per $1 000 .46 B. Voluntary Group Life Insurance, (Employee Paid) Per $1 000 Age 44 and under .30 Age 45 through 54 .78 Age 55 through 65 1.70 C. Voluntary Group Life Insurance for Dependents of Permanent City Employees (Employee Paid), Per Package of $1,000 on spouse and $1,000 on children .58 D. Group Life Insurance, $5,000 Volunteer Firemen, Per $1,000 .38; and BE IT FURTHER RESOLVED that the Director of Finance be authorized to make payroll deductions from employee's earnings for voluntary insurance coverage monthly premiums; and BE IT FURTHER RESOLVED that the Director of Finance be authorized to pay the monthly premiums for Group Life Coverage for volunteer firemen and to bill the Volunteer Fire Department for the reimbursement of premiums paid.
