PLANNED UNIT DEVELOPMENT

Notice is hereby given of a public hearing to be held by the Planning Commission of the
City of Brooklyn Center on the 12th of April 2012 at 7:00 p.m. at the City Hall, 6301 Shingle
Creek Parkway, to consider the proposed Planned Unit Development for the property described
as follows:
Address: 5401 - 69th Ave North
Legal: Lot 1 and Lot 2, Block 1, Maranatha Addition, Hennepin County,
Minnesota.
The proposed Planned Unit Development has been submitted Maranatha Conservative
Baptist Home, Inc., for rezoning of the subject site from its current R6 (Multiple Family
Residential) and R1 (One Family Residential) to a new PUD-MIXED R5/R6 (Multiple Family
Residential) district.
This PUD rezoning will also comprehend and consider a new redevelopment plan and
site and building plan for the subject site, which consists of replacing the current 97-bed skilled
nursing care center with a new 97-bed skilled nursing care addition to the existing 64-unit senior
assisted living facility (Phase 1); and includes the addition of a new 29-38 unit independent living
facility to be considered under a future Phase II.
