Three (3) copies of the following documents and information shall be submitted, at least 14 days prior to the date of the regular Commission meeting, concurrent with filing the application Irequired documents must be consistent with ordinance and policy provisions before an application may be accepted): 1. A certified site survey drawing by a registered engineer or land surveyor showing pertinent existing condition, accurately dimensioned. 2.* An accurately scaled and dimensioned site plan indicating: a) parking layouts including access provisions; b) designations and locations of accessory buildings; c) fences, walls or other screening, including heights ard type of material; d) outside lighting provisions, type and location; e) curbing. 3.* A landscape plan showing areas to be sodded or seeded; location, size and species of trees and shrubbery. 4.* Bui7djng floor plans, elevations, sections and specifications, including materials proposed ,.Y Ex;sing and ,proposed land elevations, drainage provisions, and !utility provisions. 5. Adaitronaldrawings, plans or information deemed necessary by the Secretary. *Must be prepared by a registered architect or person registered with the State Board of QgWrat on for Architects, Engineers and Land Surveyors, and said drawings/plans shall be so cortified. NOTE: Upon: approval of plans by the Council and prior to issuance of permits, a Performance Agreement as to approved site iaprovements and a supporting financ•al guarantee, in an amount to be determined by the City, are required. Acceptable financial instruments include cash escrow; certificate of deposit; and,performance bond. Copier of the Zoning Ordinance may be obtained from the Administrative Office. (Abstract-6417 Brooklyn Blvd.) "EXHIBIT A" Planning Commission Information Sheet Application No. 78065 Applicant: Aaron Hoffman Location0l56++7- Brooklyn Boulevard Request: Site and Building Review The applicant is seeking site and building plan approval for an approximate 8,800 sq. ft. office building on the Cl zoned property at 6417 Brooklyn Boulevard. The property is currently owned by the Brook Park Dental Clinic, 6437 Brooklyn Boulevard, and the applicant is in the process of purchasing the property pro- vided a parking agreement can be worked out whereby a number of parking spaces can be provided for the dental clinic on this site. Application No. 78067 submitted by Dr. Gregory Swenson contemplates a special use permit for off -site parking and should be considered concurrently with this.application. The site is a triangular shaped piece of property and can accommodate parking for 63 cars. Forty three spaces are required for the office building and 20 spaces are required for the accessory off -site parking proposed for the dental clinic. The property, at the south property line, abuts an R-1 (Single Family Residential) District, and the applicant proposes to plant a four foot high Nankin cherry hedge along the property line to provide the ordinance -required screening. The ordinance requires a four ft. high opaque fence or wall or City Council approved substitute where an Rl District abuts a Cl development. We will be prepared to review this application in more detail in conjunction with Application No. 78067. Approval of the application should be subject to at least the following conditions: 1. Building plans are subject to review and approval by the Building Official with respect to applicable codes prior to the issuance of permits. 2. Grading, drainage and utility plans are subject to review and approval by the City Engineer prior to the issuance of permits. 3. A Performance Agreement and supporting financial guarantee (in an amount to be determined by the City Manager) shall be submitted to assure completion of approved site improvements. 4. The building shall be equipped with an automatic extinguishing system to meet NFPA Standard No. 13 and shall be connected to an approved central monitoring system in accordance with Chapter 5 of the City Ordinances. 5. Any outside trash disposal facilities and/or rooftop mechanical equipment shall be appropriately screened from view. 6. All landscaped areas shall be equipped with an underground irrigation system as approved by the City Engineer. 7. Plan approval comprehends the planting of a four foot high Nankin cherry hedge as appropriate screening along the south property line in lieu of a four ft. high opaque fence or wall. 12-7-78 Application No. 78065 Page 2 8.
